This Form can be used to consider a flexible working application, this is a form and is for the Employee to complete and submit to the Employer for consideration prior to any meeting taking place.

This letter is in compliance with the Employment Relations (Flexible Working) Act 2023.

Employees have the right to apply from day one of employment, subject to eligibility.

As an Employer, you have the right to refuse – subject to criteria being followed.
